      When seeking sacramental records, such as baptism or marriage, whether for church requirements or genealogy and family history, you should contact your parish or the parish where the sacrament was administered for a certificate. A fee may be required and staff time may be limited. If further research is required, you may request records by downloading the  Application for Personal or Family History Information form. Print, fill out, sign and mail the application to:

      Please note that there is a $40.00 requested fee for researching one to five family members for genealogy or family history purposes. Please make the check to the Diocese of Fresno.

      There is no fee for sacramental information to meet church requirements.

      APPLICATIONS WITHOUT A SIGNATURE WILL NOT BE PROCESSED.
