EVENTS AND TRIPS
All policies for events and trips that are sponsored by a parish or related organization are included in the Diocesan Directives for Special Events, Groups or Organizations.  This includes an explanation of most of the forms in this section.

FORMS:

For ALL Events or Trips

• R-3 Trip and Function Request to be used when:
  • food is served at the event
  • a speaker or musicians from outside the Diocese are invited
  • advertising for the event will take place outside the parish
  • a group is being taken to an off-site location

• R-29 Lay Speaker Approval to be used when any speaker or woman religious is invited form outside the Diocese of Fresno.

• R-21 Event Adult Medical Release to be used when adults are participating in a parish/school sponsor event or field trip.

• Food-Safety-Certificate-Requirments  outlining requirements when food is prepared and served on-site
• Events where money is collected, as an entrance fee or for food or objects sold, are to use this form to for counting funds, as well as for counting the Sunday collection:  CoinandCurrencyCount

Additional forms when Minors are involved in an Event or Trip:

• R/14-15 Annual Code of Conduct Agreement is to be signed by youth and parent, agreeing to the code of conduct.

  to be signed by adults who serve as chaperone or driver for an event or trip with minors
• R-20 Event Youth Medical Release must be signed by parent/guardian for a minor to participate in an event or an off-site trip.

• R-30 Event PR Release to be signed by parent in event pictures or video are taken: 

FORMS FOR REGISTRATION IN RELIGIOUS EDUCATION OR YOUTH MINISTRY

• R-22 Annual Youth Medical Release is to be signed annually, and convers sessions of faith formation for the academic year

• R/14-15 Annual Code of Conduct Agreement is signed by minors and parent/guardian, valid for the academic year

• R-22B, R-24 Annual Online Youth Authorization and Code of Conduct is to be used for virtual classes with minors, valid for the academic year; the Code of Conduct reflects online behavior; both are to be signed by parent/guardian.
